Radiological interventions in peripheral arteries can save limb: Dr Someshwar

Arteries supply pure oxygen and nutritionally rich blood from the heart to the body parts. While veins carry away the impure, oxygen depleted blood back to the heart. Diseases of all the arteries and veins of the body other than heart are called as Peripheral Vascular Disease (PVD). If the disease happens in the peripheral arteries it is called as Peripheral Arterial Disease (PAD) and if occurs in the veins it is called as Peripheral Venous Disease, informed Dr Vimal Someshwar, Head and Chief Consultant Vascular and Interventional Radiology Department, Kokilaben Dhirubhai Ambani Hospital and Research Institute, Mumbai. He is in the city to attend symposium organised by Academy of Medical Sciences on Limbs for Life Management of Peripheral Arterio- Venous Diseases.

Diabetes is a common ailment amongst Indians, and India is considered as the diabetic capital of the world. Diabetes causes thickening of walls of the blood vessels, leading to narrowing of the lumen and hence the blood flow to the target site is reduced. This is called as limb attack. Arteries of the lower limb, especially below the knee are commonly affected. Also, in diabetics, nerves are affected and patient loose sensation and hence minor injuries goes unnoticed. The wound healing is delayed and with lack of blood supply and reduced oxygen supply, gangrene could set in requiring amputation. Diabetic patient on wearing substandard shoes or sandals can easily suffer from shoe bites. With underlying diabetes, these shoe bite wounds do not heal inspite of medication and minor surgical dressings. The wound and adjacent tissue soon become black and painful. This is pre-gangrene, which can soon become gangrene resulting in amputation, sepsis or even death. Apart from diabetes, atherosclerosis, thrombosis, artery inflammation can cause similar problems. This condition needs urgent management by restoring blood flow. Interventional radiology has now revolutionized the treatment of such conditions. Being the most advanced branch of modern medical science it provides complete information about the blocked arteries by peripheral angiography, and offers treatment options of opening up of these blocked arteries by angioplasty, stenting and thrombolysis. Dr Vimal Someshwar, suggested Life Style Changes like balanced exercise with rest, cessation of smoking, foot care, low-cholesterol and low-fat diet, control of obesity, and regular medical check up for diabetes and hypertension for prevention of PVD. He also emphasized on blood pressure check up especially of legs and calculation of ankle brachial index, atleast once a year can lead to early detection of disease. He reiterated that Team Approach is necessary to treat such patients and hence specialized Diabetic Foot Clinic and Peripheral Vascular Disease Clinic is need of time and awareness amongst patients and family physicians is necessary to avoid amputation and salvage limbs as far as possible.
